In 1963, Dr. Middleton graduated from the University of Utah with a Bachelor of Science. While attending this university, he was the president of the fraternity, Beta Theta Pi, as well as chairman of U-Days. Dr. Middleton continued his studies at Cornell University Medical College, receiving a MD in 1966. He was elected to Alpha Omega Alpha during this time.
Following her graduation, Dr. Middleton was an intern at the University of Utah, College of Medicine. In 1967, he was a resident in general surgery at Cornell/New York Hospital. Dr. Middleton served as a captain in the U.S. Air Force between 1968 and 1970. Later in 1974, he became a resident in urology at Harvard Medical School and Massachusetts General Hospital.
Since 1980, Dr. Middleton has worked as a clinical associate professor at the University of Utah and faculty member of the division of urology. Since 2009, he also has been a member of the admissions committee of this same university. Throughout his career, Dr. Middleton has contributed as an author of scientific papers to his field. He has also served as a mission president of the Church of the Latter Day Saints in Vancouver from 2005 to 2008. In 2017, he co-authored "Vancouver Mission Memories, 2005-2008" alongside Carol Middleton.
As an esteemed urologist and educator, Dr. Middleton is a member of various related organizations, including the American Urological Association, American Association of Clinical Urologists, Utah Medical Association, American Medical Association, Salt Lake County Medical Society, Salt Lake Surgical Society, and American College of Surgeons. Dr. Middleton was a founding member and the first chairman of the board of UROPAC between 1992 and 1995. He was a delegate to the AMA, as well as a national president of AACU, the malpractice insurance company, and was elected to board governor of UMA.
He was the medical director, of Uroquest and Prime Medical. Dr. Middleton received a Distinguished Service Award from Utah Medical Association in 1993, an Outstanding Service Award from the western section of the American Urological Association in 2005, and from the national American Urological Association in 2011. Dr. Middleton has been elected president of the following organizations: the Utah Urological Society from 1975 to 1976, the Salt Lake County Medical Society from 1983 to 1984, the Utah Medical Association from 1987 to 1978, the national AACU from 1991 to 1992, the western section of the American Urological Association from 2000 to 2001, the Timpanogos Club from 2004 to 2005, the Collegium Aesculapium international Latter Day Saints physicians' organization from 2011 to 2012, and the emeritus alumnus board of the University of Utah from 2014 to 2015.
